J Mendez Restoration LLC; 25 Briarwood Ln, New Wilmington, PA 16142: Residential roof- on or about 09/12/2024 and at times prior thereto, employees were conducting roofing operations on a residential roof with a 12:12 pitch without any fall protection systems. Employees were exposed to fall hazard of 10-feet above ground level. Employees were observed not connected to the vertical lifeline while walking the roof.

J Mendez Restoration LLC; 25 Briarwood Ln, New Wilmington, PA 16142: Residential roof- on or about 09/12/2024 and at times prior thereto, employees were installing shingles on a roof with a 12:12 pitch. Employee was observed using a body belt as a means of fall protection and were exposed to a fall arrest condition with a 10-foot fall hazard. The body belt used by employees did not attach to the wearers back.
