29 CFR  1926.404(b)(1): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.   a)  224 North Liberty Street, Rome, NY  13440, on the ground,  on or about 9/11/24:  Workers were cutting dimensional lumber for use in rebuilding the roof structure, using a  DeWalt circular saw without ground fault circuit interruption protection.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  224 North Liberty Street, Rome, NY  13440, on the second floor deck, on or about 9/11/24:  Workers were rebuilding a roof structure at 26 feet above ground without fall protection.
