29 CFR 1910.23(e)(2)(i): The employer did not ensure that steps are uniformly spaced and arranged, with a rise of not more than 10 inches (25 cm) and a depth of not less than 7 inches (18 cm). The employer did not ensure that the slope of the step stringer to which the steps are attached is not more than 60 degrees, measured from the horizontal.  On or about August 22, 2024, an employee utilized a mobile platform ladder that had been altered from the original design exposing the employee to a fall hazard. The mobile platform ladder was equipped with an additional step on the platform exceeding 10 inches and the slope of the step was more than 60 degrees.
