Location: Job site; On or about 09/13/2024, employees were allowed to engage in roofing activities without the use of fall protection on a residential roof that was 20-feet or more above the ground below.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. Location: Job site; On or about 09/13/2024, employees were allowed to utilize temporary roof anchors which were not installed per manufacturer instructions to support 5,000lbs per employee. Instance a) multiple anchorage points observed not installed per manufacturer instructions and had multiple fasteners missing. Instance b) an anchorage point was observed and documented to be completely unsecured on one half and was flipped over onto itself and visibly not touching any portion of the roof. A lifeline is also observed as being attached to this unsecured anchorage point.
