29 CFR  1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    On or about September 13, 2024, employees were exposed to electrical hazards when assigned to trim trees near energized overhead power lines. The employer did not train employees and ensure they were familiar with safety-related work practices addressing electrical hazards related to tree trimming near unprotected energized 7200 V overhead electrical power distribution line.

29 CFR  1910.333(c)(3): When work was to be performed near overhead lines, the lines were not de-energized and grounded, nor were other protective measures provided before work was started.  Where lines are to be de-energized, arrangements were not made with the person or organization that operates or controls the electric circuits involved to de-energize and ground them.  On or about September 13, 2024, employees were exposed to electrical hazards when trimming a tree near energized unguarded overhead power lines. The employer did not arrange with the power distributor for the overhead power lines to be de-energized and grounded, nor did the employer institute other protective measures before tree trimming work started. An employee trimmed with a manual aluminum pole saw 9 feet, 9 inches from an energized unguarded 7200 volts overhead power distribution line.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact came closer than 10 ft. (305 cm) to any unguarded, energized overhead line:  On or about September 13, 2024, an unqualified employee was exposed to electrical hazards posed by a 7200-volt overhead power distribution line while tree trimming. The employer failed to ensure the employee elevated in a tree was located such that neither the employee nor their long conductive pole saw would contact or come closer than 10 feet from the unguarded energized overhead line or tree limbs contacting the power line. The elevated employee used a pole saw extended greater than 20 feet while he was approximately 9 feet, 9 inches from the overhead line when they contacted an energized limb or power line.

The employer did not provide employees with electrical protective equipment including but not limited to insulated gloves for hands when trimming trees near an unprotected energized 7200 volts overhead electrical power distribution line.

29 CFR  1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts:  On or about September 13, 2024,  an employee was performing tree-trimming operations near overhead power lines with a non-insulated saw, exposing him to electrical hazards.
