29 CFR 1926.1211(b)(4):  The employer did not ensure that affected employees practice making permit space rescues before attempting an actual rescue, and at least once every 12 months, by means of simulated rescue operations in which they remove dummies, manikins, or actual persons from the actual permit spaces or from representative permit spaces:      On or about September 11, 2024, and at times prior thereto, employees conducted construction operation in manhole permit required workplaces, and the employer did not ensure the affected employees practiced permit space rescues prior an actual rescue at least once every 12 months.
