On or about September 12, 2024, during the installing of roofing material on a residential structure at 6718 Parma Park DR, Parma Heights Ohio 44130, the employer did not provide or require eye protection to be worn while employees were using pneumatic nail guns during shingle fastening.

The employees performing roofing installation on the structure were exposed to falls of 21 to the ground below.
