29 CFR 1910.269(e)(7): The employer did not ensure that while work is being performed in the enclosed space, an attendant with first-aid training was immediately available outside the enclosed space to provide assistance if a hazard exists in the area of the opening used for entry, including requirements in paragraph (t) of this section for additional requirements on attendants for work in manholes and vaults. a) Jobsite: 4200 City Avenue Philadelphia, PA - Manhole 144520 - On or about September 16, 2024, the employer did not ensure that an employee performing underground electrical work was protected from injury by use of a manhole attendant. Two employees were preparing to test electrical phases. One employee was located at the rear of a work truck while the other employee entered the manhole and sustained injuries related to an electrical flashover.
