29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: On or about September 16, 2024, employees working on or around were exposed to fall hazards, including but not limited to the following: b. Employees working by the autopour operator platform were exposed to fall hazards of approximately 6 feet. c. Employees working on the melting deck were exposed to fall hazards because railing was missing on the deck.

29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: On or about September 16, 2024, employees working on or around the autopour enclosure were exposed to burn hazards because an employee was not protected from falling into the enclosure during dross skimming activities.
