On or about 9/17/2024, lockout/tagout procedures were not developed for the Peddinghaus Advantage 2 Three Spindle Drill and the MEBA Steel 1250 DG miter band saw ("Drill Saw Tandem").

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Location - Warehouse / service floor. On or about 9/6/2024, the MEBA Steel 1250 DG miter band saw was not guarded to protect employees from the moving parts on the machine, exposing employees to amputation and laceration injuries.
