29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) 10405 U.S. 522 South, Lewistown, PA 17044 - On or about September 17, 2024, the employer did not establish an energy control program consisting of an energy control procedures, periodic inspections, and training for employees who conducted the cleaning of the Bizerba Deli Slicer (Model GSPH33).

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) 10405 U.S. 522 South, Lewistown, PA 17044 - On or about September 17, 2024, employees who conducted cleaning of the Bizerba Deli Slicer (Model GSPH33) did not unplug the electrically powered machine to isolate from the energy source, before removing the guards to the slicing blade. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
