29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:     (a) On September 16, 2024 at or near 313 North 26th Street in Bismarck, North Dakota, employees were exposed to fall hazards of over 6 feet while engaged in roof sealing operations.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) On September 16, 2024 at or near 313 North 26th Street in Bismarck, North Dakota, employees were exposed to hazards associated with ladder displacement while using a portable extension ladder to access a roof where the ladder extended less than 1 foot above the landing surface.
