29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) Hanger 21: On or about May 10, 2024, the fall protection system trolly had inadequate bumper guards and fell of the system while 3rd MXS employee(s) were conducting assigned work and the system was not removed from service, exposing employees to struck by and fall hazards.
