(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    Guardrail systems; Safety net systems; or Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  On or about September 17, 2024, at an establishment located at 212501 State Highway 97 in Stratford, WI employees were performing residential roofing activities with no form of conventional fall protection while exposed to fall hazards of over 9 feet.

29 CFR  1910.213(c)(1): Each circular hand-fed ripsaw shall be guarded by a hood which shall completely enclose that portion of the saw above the table and that portion of the saw above the material being cut. The hood and mounting shall be arranged so that the hood will automatically adjust itself to the thickness of and remain in contact with the material being cut but it shall not offer any considerable resistance to insertion of material to saw or to passage of the material being sawed. The hood shall be made of adequate strength to resist blows and strains incidental to reasonable operation, adjusting, and handling, and shall be so designed as to protect the operator from flying splinters and broken saw teeth. It shall be made of material that is soft enough so that it will be unlikely to cause tooth breakage. The hood shall be so mounted as to insure that its operation will be positive, reliable, and in true alignment with the saw; and the mounting shall be adequate in strength to resist any reasonable side thrust or other force tending to throw it out of line.  On or about September 17, 2024, at an establishment located at 212501 State Highway 97 in Stratford, WI employees were exposed to caught in hazards while operating a Grizzly G1023SLX Left-Tilting 10" table saw with a guard that did not completely enclose the saw blade above the table exposing employees to struck by and amputation hazards.

29 CFR  1910.213(c)(3): Each hand-fed circular ripsaw shall be provided with nonkickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator. They shall be designed to provide adequate holding power for all the thicknesses of materials being cut.  On or about September 17, 2024, at an establishment located at 212501 State Highway 97 in Stratford, WI employees were exposed to caught in hazards while operating a Grizzly G1023SLX Left-Tilting 10" table saw that did not have nonkickback fingers installed exposing employees to struck by and amputation hazards.

On or about September 17, 2024, at an establishment located at 212501 State Highway 97 in Stratford, WI employees were exposed to shock and fire hazards while utilizing receptacles on the middle and south lines that had damage from overheating.

29 CFR  1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.  On or about September 17, 2024, at an establishment located at 212501 State Highway 97 in Stratford, WI employees exposed to electrical and fire hazards by not removing damaged electrical cords from service in the following areas:   a)  In the roofing department the employer did not protect employees and remove a damaged extension cord being utilized to operate a miter saw.  b)  On the south line, the employer did not protect employees and remove a damaged plug and cord set from service.
