29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) On or about September 17, 2024, at a jobsite located at 3101 East Lindy Lane, Oak Creek, WI 53154, an employee was working on a ladder jack scaffold without fall protection. This exposed the employee to a fall hazard of approximately 16.8 feet to the lower level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 17, 2024, at a jobsite located at 3101 East Lindy Lane, Oak Creek, WI 53154, employees conducting roofing activities (shingle installation) on a 9:12 pitch roof of the residential structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the roof of the structure of approximately 24 feet on the second level and approximately 19 feet on the first level of the house.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about September 17, 2024, at a jobsite located at 3101 East Lindy Lane, Oak Creek, WI 53154, the employer did not ensure that employees exposed to potential fall hazards had been properly trained to recognize, minimize, or eliminate the fall hazards associated with this work. This exposed employees to a fall hazard of approximately 24 feet feet onto grass, building materials, and concrete.
