29 CFR 1926.451(f)(15):Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. Location: a) Exterior Supported scaffold at the jobsite: On or about September 16, 2024, an employee used a 20-foot extension ladder, that was placed on top of a sidewalk shed scaffold platform, to loosen a construction material hoist rope that was stucked to an air conditioner component. When the employee climbed up, he fell because the ladder was not setup properly to prevent against sideways thrust or the ladder legs from slipping. The employee was exposed to falling up to twenty feet.
