29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A) through(c)(4)(ii)(D): a) 939 Monocacy Rd, York, PA 17404, thermoplastic production line, Mill #5 - The employer developed two different written energy control procedures, (Lock Out/Tag Out! Thermoplastics Mills 5 and 6 and Cleaning Procedures for Cryogenic Mills) for the changeover and cleaning of the machine. These procedures did not provide techniques for the notification of employees, lockout of valves, verification of isolation, and release from lockout / tagout. This exposed employees to uncontrolled hazardous energies of moving machine parts and extreme cold temperatures, on or about September 18, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) 939 Monocacy Rd, York, PA 17404, thermoplastic production line, Mill #5 - Supervisors did not enforce sequential steps including, but not limited to, locking out of valves and verification of isolation, which were required for the energy control of Mill #5 while maintenance staff and machine operators routinely preformed troubleshooting and clearing of jammed production materials. This practice exposed employees to the uncontrolled hazardous energies of moving machine parts and extreme cold temperatures, on or about September 18, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
