On or about 08/01/2024, at 405 Tres Lagoas Street, Punta Gorda Florida, 33983, an employee was operating a dump truck in reverse with an inoperable backup alarm and without an observer/ground guide, during which time two employees were positioned outside of their vehicle while performing maintenance on a track vehicle; therefore exposing the employee to a vehicular struck-by hazard.
