29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Location: 2137 5th Ave, Ronkonkoma, NY 11779. On or about September 18th, 2024, an employee was operating a CNCC electric forklift to move cardboard boxes while two other employees were sitting on the boxes. One of the employees fell off the boxes as the forklift was moving. The operator was not trained in the safe operation of the forklift Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
