29 CFR 1910.22(a)(2):The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition: Worksite located 2727 Partnership Boulevard, Springfield, Missouri- Line #9 and other lines- Employees were exposed to slip and fall hazards near the machines in which hydraulic fluid had leaked all over the floor areas.

29 CFR 1910.212(a)(2):Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: Worksite located at 2727 Partnership Boulevard, Springfield, Missouri- Lines #5, #9, & #14- The thermal guard on the plastic injection machines had been removed and not replaced in location in which employees were subject to burn hazards from the high temperatures.
