A) 44 Woodlawn Avenue, Yonkers - NY:  There was debris and lumber with protruding nails all over the ground in which the employees were walking on, on or about, September 14th, 2024.

The extension ladder that was used, did not extend at least 3 feet above the landing, on or about, September 14th, 2024.
