29 CFR 1926.651(b)(1): The estimated location of utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, shall be determined prior to opening an excavation. a) 1309 Crist Dr, Billings, MT, 59105: On or about September 20, 2024, and at times prior, employees were exposed to hazards such as, but not limited to, fire, explosion, and shock hazards while digging a trench to repair a broken sewer line. The employer failed to conduct line and utilities locates to identify lines such as gas and underground electrical lines prior to digging the trench.

In the event the trench started to collapse, employees were required to use an A-Frame ladder that was approximately 24 inches below the top of the excavation to exit the trench.
