29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  a)	Laboratory and Exam Rooms:  On or about September 23, 2024, and at times prior,  the employer did not document by written certification that the required worksite hazard assessment had been performed where employees were provided with and used personal protective equipment including nitrile gloves, gowns and voluntary use N95 respirators while drawing blood and processing blood samples.
