29 CFR 1926.350(a)(3): Cylinders were moved by tilting and rolling them on their bottom edges. They were intentionally dropped, struck, or permitted to strike each other violently. (a) C&C Plumbing, Jobsite, Bethlehem, CT; On or about, September 20, 2024, and at times prior, an employee repeatedly struck a MAP Pro canister on the ground resulting in a crack of the canister and an escape of gas to the atmosphere.

29 CFR 1926.350(g)(2): Torches in use were not inspected at the beginning of each working shift for leaking shutoff valves, hose couplings, and tip connections. Defective torches were used. (a) C&C Plumbing, Jobsite, Bethlehem, CT; On or about, September 20, 2024, and at times prior, employees did not remove defective torches before conducting MAP gas soldering on a boiler. An employee attempted to dislodge a blockage in a torch by striking the cylinder on the ground which resulted in an expulsion of gas from the canister that caught fire with another lit torch.
