On or about September 23rd, 2024 Location: 185 Milton Rd, Rye, NY 10580 a) Employee was exposed to approx. 13 ft fall while working on a pump jack scaffold without fall protection. Toprail for a pump jack scaffold was set up to a height of approx 5 ft from the working platform.
