OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  a) Production Department, Entrance: On August 1, 2024, and at times prior, the employer exposed employees to struck-by hazards in that the lower ends of two storages racks, located immediately across from the entrance into the department, had bent and damaged supports. Employees operated powered industrial trucks at and around them.  b) Production Department, Between Shelves labeled "OVN-D23-B-B" and OVN-D24-B-A":  On August 1, 2024, and at times prior, the employer exposed employees to struck-by hazards in that a storage rack had had bent and damaged supports. Employees operated powered industrial trucks at and around them.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Department, Accu-Cut: On August 1, 2024, the employer exposed employees to amputation hazards, in that the employer did not implement a method of machine guarding to prevent employees from contacting pinch points created by guide belts moving the automated rollers at the outlet of the Accu-Cut Q-9 machine as employees rolled and removed cut material from the machine during operation.  b) Production Department, Fabric Rolling Machine: On August 1, 2024, and at times prior, the employer exposed employees to amputation hazards, in that the employer did not implement a method of machine guarding to prevent employees from contacting pinch points created by the two sets of automated rollers that make up the fabric rolling machine as employees sat between them to ensure that material was properly moving through the machine.
