1910.147(c)(4)(ii)(B) The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about September 23, 2024, and at times prior thereto, the energy control procedures for plastic mold injection plant machines and equipment were not developed and used by employees for the control of hazardous energies, while performing servicing, cleaning and unjamming operations.

29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment shall contain an equipment grounding conductor: On or about September 23, 2024, and at times prior thereto, in the Mold Injection Plastic Presses production department, employees used equipment that had a damaged electric flexible cord with missing grounding conductor that affected the safe operation of the machine system while conducting plastic parts production.
