Archer Mills Phase 6. On or about September 24, 2024, in a trench greater than 4-feet deep, the employer did not provide a stairway, ladder, ramp or other safe means of egress, exposing employees to emergency egress hazards.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary: a. Archer Mills Phase 6. On or about September 24, 2024, in a trench with vertical walls, the employer exposed employees to struck-by hazards and engulfment hazards from unstable soil spoil piles placed at the top leading edge of the vertical sidewalls of the trench with excavated soil and debris sliding back into the trench.
