Surface Solutions Consulting, LLC Jobsite: 23-30 Borden Ave, Queens, NY. a) The employer did not test and ensure a shop made extension cord to power the 480VAC 3 Phase SASE PDG 8000 Planetary Diamond Grinder and Pullman Ermator T8600 Dust Extractor was free from hazards such as but not limited to cut insulation, loose wire termination connections and uninsulated wires in the Hubbell Connectors. An employee was plugging in a male and female connector and was burned when an arc flash occurred; on or about 09/20/2024. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
