29 CFR 1910.133(a)(3): The employer did not provide protective eyewear for employees who required prescription glasses while engaging in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:  a) Shop:  An employee wore prescription glasses without the use of safety glasses while exposed to flying particles and objects when operating equipment, such as, but not limited to, a pneumatic nail gun, compressed air for cleaning and a chop saw, on or about 9/25/24.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1910.157(g)(1):An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  a) Throughout facility:  Employees required to use fire extinguishers to fight incipient stage fires, were not provided training on general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting, on or about 9/25/24.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

A): Shop: The employer failed to provide initial forklift training to an employee who was operating the Komatsu forklift model FG30SHT-12 that consisted of formal classroom training, a driving test and a written evaluation, on or about 9/25/24.
