29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 25, 2024, it was observed that employees engaged in residential roofing for Laurencio Cuevas Hernandez at 4920 Curlew Lane, Omaha, Nebraska were exposed to fall hazards of approximately 9 feet 4 inches as measured from the ground to the eave in that the employees were re-roofing the home while standing on the roof without any form of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about September 25, 2025, it was observed that employees engaged in residential roofing activities for Laurencio Cuevas Hernandez at a residential construction site located at 5920 Curlew Lane, Omaha, Nebraska were exposed to fall hazards of approximately 9 feet 4 inches in that the employer failed to provide employees with training necessary in recognizing potential hazardous fall conditions and on the use of fall protection.
