On or about 09/24/2024, at Ona Polymers, the employer did not perform tests on Vinyl Acetate Tank (M-1) consistent with applicable manufacturers' recommendations and good engineering practices, such as but not limited to American Petroleum Institute 510 and more frequently if determined to be necessary by prior operating experience.
