29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about September 26, 2024, and at times prior, the employer had not utilized the hazardous energy control procedures for the Spartan bandsaw (Model - PA18/2, Serial - FPA180237) to isolate energy source when changing out saw blades. This exposed saw operators to a caught-on and amputation hazard.

29 CFR 1910.147(d): At this establishment, the established procedures for the application of energy control (the lockout and tagout procedures) did not cover the following six elements in the following sequence: (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished: (a) On or about September 26, 2024 and at times prior, the employer had not utilized the hazardous energy control procedures for the Spartan bandsaw (Model - PA18/2, Serial - FPA180237) to isolate energy source when changing out saw blades. This exposed saw operators to a caught-on and amputation hazard. The employer failed to implement energy control application steps as the machine(s) were not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolating. (a) On or about September 26, 2024, and at times prior, the employer failed to train and instruct employees who are authorized to perform blade changes on the Spartan bandsaw (Model - PA18/2, Serial - FPA180237) to the authorized level. The saw operators were trained to the affected employee level but were tasked to perform blade changes that require lockout tagout procedures to be implemented.
