On or about 09/26/2024 and at times prior thereto; Employees used a 6-foot shock absorbing lanyard with a pelican style snaphook that did not operate correctly. The pelican snaphook self-locking keeper did not fully close and was not removed from service prior to use.

On or about 09/26/2024 and at times prior thereto; employees were exposed to hazards associated with material being dropped from the top of the 47-foot building without a chute. Materials being dropped from the roof include, but is not limited to, 2X4 lumber, exterior insulation foam system (EIF), and fascia boards.
