29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Facility: On or about September 30, 2024, the employer exposed employees to a fall hazard, in that employees were not rigged to prevent contact with a lower level, including facility equipment inside the building. b) Facility: On or about September 30, 2024, the employer exposed employees to a fall hazard, in that employees were not rigged to prevent free fall more than 6 feet when using a Shockwave lanyard anchored at foot level.
