OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards while operating two ten-ton underhung overhead cranes.  On or about October 1, 2024, the employer did not ensure that employees were protected from struck-by hazards when lifting loads with two ten-ton under-hung overhead cranes that were not inspected. If the system failed, employees could be struck by falling objects hoisted into the air. The employer's last inspection records on file were for January 24, 2019.  ASME B30.17-2020- Cranes and monorails (with underhung Trolley or Bridge), ASME B30.20-2021-Below the hook lifting devices, Federal Specifications: RR-W-410D, ASME B30.9, API 9A, API RP 9B, API RP 2d, OSHA standards for industrial slings. The employer should conduct a pre-shift inspection to frequent and periodic inspections.
