On or about 09/27/2025, an employee in the fabrication shop was exposed to amputation hazards while operating a Pistorius Saw model SC-16P serial: 45734. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. Site: 925 Waverly Avenue, Holtsville, NY, 11742 On or before 09/27/24, The employer did not notify OSHA within 24 hours of a work-related incident that resulted in an employee amputation of left index finger. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
