At the jobsite located at 1638 Southeast Decker, Lees Summit, MO 64081: Employees are tasked with moving pieces of metal onto the conveyor belt of a Nexgen Baler. The employer did not provide any means of fall protection to protect employees from falling onto the conveyor belt which leads to the baler.
