29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Employees engaged in grinding activities at the Jet 8-inch pedestal grinder in the center of the south shop were exposed to struck-by hazards in that distance between the abrasive wheel and the tongue guard was found to exceed one fourth inch. It was measured at 3/4 inches.

29 CFR 1910.134(c)(2)(i): The employer did not provide respirator users with information contained in Appendix D to this section ("Information for employees using respirators when not required under this standard") when voluntary respirator use was permissible: Employees engaged in manufacturing activities while voluntarily using respirators (dust masks) were exposed to health hazards in that the employer had not determined that the respirator itself will not create a hazard. The information contained in 29 CFR 1910.134 Appendix D was not provided for employees voluntarily using the Condor Utility Dust Mask 6140092, 2KFY4M respirators while performing polishing or deburring tasks.
