(a) On or about October 1, 2024, at 9936 S. Shepard Ave, Oak Creek, WI 53154, employees installing a roof on an existing residential home with an approximately 7:12 pitch roof were not using fall protection. This exposed employees to a fall hazard of approximately 9 feet 4inches from the roof eve to the ground below on the front (west) side of the home.
