On or about October 01, 2024, during a commercial construction roofing project located at 4328 CRANWOOD PKWY, WARRENSVILLE HEIGHTS, OH 44128, the employer did not ensure employee(s) engaged in roofing work activities demonstrated an adequate knowledge of the fall protection requirements to identify and implement conventional or personal fall protection while exposed to fall hazards in the work area.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about October 01, 2024, during a commercial construction roofing project located at 4328 CRANWOOD PKWY, WARRENSVILLE HEIGHTS, OH 44128, The employer did not ensure that employee(s) working from a low-sloped roof were protected from fall hazards by the use of a guardrail system, safety net system or personal fall arrest system and/or combination warning line system and safety monitoring system. TUSING BUILDERS & ROOFING SERVICES. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1556309, citation number 01, item number 001 and was affirmed as a final order on 11/10/2021, with respect to a workplace located 2097 E.
