29 CFR  1926.1404(d)(1):Before commencing assembly/disassembly operations, the A/D director must ensure that the crew members understand all of the following:  On or about September 26, 2024, at a worksite located at 5469 Deerfield Avenue, North Lawrence, Ohio, the employer did not ensure millwright employees working as crew members commencing in the assembly and disassembly operation of a swing-away boom extension, also known as a jib, of a rented Grove TMS500E 50-ton mobile crane were given crew instructions by an Assembly/Disassembly (A/D) Director to ensure the millwright employees understood their tasks, the hazards associated with their tasks, and the hazardous positions/locations they needed to avoid.  The millwright employees were not provided with instruction to ensure they understood the assembly/disassembly tasks that included, installing and removing the left and right side attachment pins of the boom extension and the operation of the boom extension rear stowage bracket handle and hitch pins [per 1926.1404(d)(1)(i], the hazards associated with installing and removing the left and right side attachment pins of the swing-away boom extension and the operation of the boom extension rear stowage bracket handle and pins [per 1926.1404(d)(i)(ii)], and the hazardous positions/locations they needed to avoid such as, working from a ladder under the boom extension to remove the left and right side attachment pins and on the crane deck under the boom extension near rear stowage bracket [per 1926.1404(d)(1)(iii)].

29 CFR  1926.1404(f)(1):When pins (or similar devices) are being removed, employees must not be under the boom, jib, or other components, except where the requirements of paragraph (f)(2) of this section are met.  On or about September 26, 2024, at a worksite located at 5469 Deerfield Avenue, North Lawrence, Ohio, the employer did not ensure the two millwright employees assisting in the disassembly of the swing-away boom extension, also known as a jib, of a rented Grove TMS500E 50-ton mobile crane, were not under the swing-away boom extension when the left and right side attachment pins of the swing-away boom extension were removed.

29 CFR  1926.1427(f)(1): Through an evaluation, the employer must ensure that each operator is qualified by a demonstration of:  On or about October 2, 2024, the employer did not ensure an evaluation of each crane operator was conducted and/or documented to ensure crane operators were qualified to operate mobile cranes, by a demonstration of their skills and knowledge of operating the crane equipment safety.  Crane operators operate mobile type cranes owned by the employer from there Upper Sandusky, Ohio, Attica, Ohio, and Napoleon, Ohio facilities.
