29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about Sept. 25, 2024, at 2924 South Brentwood Boulevard in Brentwood, MO, employees were exposed to fall hazards while working around a hole in a roof without fall protection.  Employees were removing unstable decking section of roof when an employee fell through the bad decking that was unstable and landed on the floor of inside of the building, approximately 15 feet, below.
