OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that workers were exposed to the hazards associated with induction furnace explosions: On or about October 1, 2024, and at times prior thereto, at the LA Induction Furnace, employees were exposed to an explosion and sudden release of molten metal while making multiple attempts to prevent a bridging condition including tilting of the furnace.

29 CFR 1910.23(b)(8): Ladders were not used only for the purpose for which they were designed. On or about September 2024, and at times prior thereto, next to the LA Induction Furnace console, an employee used only the lower piece of an extension ladder to ascend/descend into a furnace to perform an inspection and patch work for the refractory.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. On or about October 1, 2024, and at times prior thereto, at the LA Induction Furnace area, the employer failed to ensure personal protective equipment necessary for hot, molten metal conditions was used: a) The melting operator was not wearing fire-retardant clothing and sleeves for work around molten metal. b) The plate setter was not wearing fire-retardant clothing for work around molten metal. c) The pourer was not wearing aluminized safety hood, gloves, and leggings or spats for work around molten metal.
