29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about October 2, 2024,  the employer did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about October 2, 2024, employees conducting tear off work on the second story of a residential roof with a 4:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 17 feet from roof to the ground below.  (b) On or about October 2, 2024, employees conducting tear off work on the north side and south side of the first story of a residential roof with a 4:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately nine (9) feet from roof to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about October 2, 2024, employees installing shingles on an existing residential home, used a Gorilla Aluminum ladder to access the roof that was not erected three (3) feet above the upper landing.

29 CFR  1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall.  (a) On or about October 2, 2024, an employee was using an aluminum extension ladder while carrying shingles up on his shoulder to reach the second story, exposing the employee to a fall hazard off the ladder onto the concrete below.
