Each employee engaged in residential construction activities 6-feet or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest systems.   A) 44 Woodlawn Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 19 (back roof) and 25 1/2-feet (front/side roof) while engaged in solar panel installation work on a structure, without a means of fall protection in place, on or about, October 1st, 2024.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 44 Woodlawn Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 9 and 19-feet while using extension ladders to access two different levels of the roof. The extension ladders that were used, did not extend at least 3 feet above the landing, on or about, October 1st, 2024.
