On or about October 2, 2024, during the installation of roofing material at the job site located at 3673 Massillon RD, Uniontown Ohio 44685; the employer did not ensure employees working from a ladder jack scaffold 14.1 feet above the ground were protected from falling by a guardrail, personal fall arrest, or other fall protection system.

29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about October 2, 2024, during the installation of roofing material at a job site located at 3673 Massillon RD, Uniontown Ohio 44685; the employer did not ensure employees working on a roof of a commercial location were protected from a fall of approximately 17.5 feet by utilizing a fall protection system. The CAMPOPIANO ROOFING CO was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1618278, citation number 01, item number 01a and was affirmed as a final order on 10/13/2022, with respect to a workplace located at 2726 Hudson DR, Cuyahoga Falls Ohio 44221.
