On or about October 3, 2024, MR CONSTRUCTION & ROOFING LLC did not ensure that six employees roofing a residential building at 2046 St. Paul Road in St. Paul, Missouri, utilized fall protection, thus exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): When portable ladders had been used for access to an upper landing surface, the ladder side rails had not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder had been used to gain access. On or about October 3, 2024, MR CONSTRUCTION & ROOFING LLC did not ensure that a ladder used by six employees to access the roof of a residential building at 2046 St. Paul Road in St. Paul, Missouri, was extended at least three feet above the upper landing surface, thus exposing employees to fall hazards.
