29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in �� 1926.502 (Appendix G to this subpart). At the jobsite located at 2715 Industrial St., Junction City, Kansas, employee(s) engaged in roofing work on a steel structure were exposed to fall hazards in that employee(s) were not utilizing fall protection properly while working approximately 17 feet in height. On 10.1.2024, an employee who was wearing a personal fall arrest system and a lifeline slipped at the peak of the roof, fell to the edge, and then fell off the roof. The employee(s) life line was not rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level; and was not rigged such that the system would bring an employee to a complete stop and limit maximum deceleration distance an employee travels to 3.5 feet (1.07 m).

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761: At the jobsite located at 2715 Industrial St., Junction City, Kansas, employee(s) engaged in roofing work on a steel structure were exposed to fall hazards in that employee(s) were not utilizing fall protection properly while working approximately 17 feet in height. Employee(s) were not utilizing their anchors and their lifeline systems properly and did not know the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used.
