29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  Location: 150 Sutton Street, North Andover, MA  a) On or about 10/3/24, the employer did not ensure that employees working on a pitched roof were attached to fall protection equipment. An employee at the peak of the roof was wearing a personal fall arrest harness, but was not attached to fall protection equipment on the roof.

29 CFR  1926.502(d)(17):The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  Location: 150 Sutton Street, North Andover, MA  a) On or about 10/3/24, the employer did not ensure that employees working on a pitched roof were attached to fall protection equipment at the attachment point on the center of the wearer's back. Employees attached a lanyard to the front of the personal fall arrest harness.

Employees were attached to the fronts of lanyards, and were improperly wearing personal fall arrest harnesses.
